  • Copies of your current policy(ies)
  • When should your new coverage take effect?
  • The vehicle identification number (VIN) for vehicles to be insured
  • Drivers' names, license numbers, birth dates and social security numbers
  • Grade point averages of drivers who are high school or college students
  • Garage locations of all vehicles to be insured
  • The proximity of your local fire department and fire hydrant to your home
